Learning Collaborative

Collaborative learning occurs when a group of individuals or organizations share information and experiences in an effort to drive change and seek improvement for a common purpose. The RHP 8 Learning Collaborative seeks to achieve regional impact through shared learning, community engagement, and celebration of success. We believe there can be great benefit to the region at large when stakeholders come together in activities designed to foster collaboration and dialogue geared toward learning and continuous quality improvement.

The design of the RHP 8 learning collaborative is a version of the CMS-recommended IHI model, modified to accommodate our smaller, more rural region. The collaborative structure is very similar to that of our sister region, RHP 17 in College Station, and a larger regional counterpart in Houston, RHP 3, to allow participation across regions by interested providers.

"Individual" and "Regional" mechanisms for learning and sharing, fed by "Core" components, work in a continuous cycle to promote quality improvement and positive regional impact as illustrated below.

Learning Opportunities

Regional Opportunity:

The Anchor team invites you to visit our new website. We hope you are able to explore the new pages made available that showcase work being done in the RHP 8 Cohort, along with hearing updates and best practices on DSRIP projects in the region that are shared via the monthly conference call.

We have also added a Google calendar that showcases the various learning collaborative events.

Peer-to-Peer Opportunity:

RHP 6 is hosting a Primary Care Learning Collaborative Event: A DSRIP Road Trip with four stops throughout DY 4. The first stop will be at the Uvalde Memorial Hospital in Uvalde on November 19, 2014, from 10:00 a.m. – 2:00 p.m. Various RHP 6 DSRIP Providers will present the impact their Community Health Worker (CHW) programs have had thus far. Register for the first stop on the tour or visit RHP 6’s website for more information.

Regional Work Group

RHP 8 Behavioral Health & Primary Care Cohort

The RHP 8 Behavioral Health & Primary Care Cohort aims to bring together DSRIP Providers and other RHP stakeholders who have similar projects, goals, and interests in identified focus areas.

The objective of the Cohort is to foster collaboration through focused quality improvement activities in an effort to promote efficiencies and improve healthcare delivery and access throughout the region. Learn more about the Cohort.
