Learning Collaborative

Many projects in RHP 8 test inventive models of care that move away from historical systems of treatment, and toward a more integrated and coordinated approach. These inventive models focus on the patient’s whole-person needs and health outcomes. As RHP 8’s Performing Providers implement advances, it is important they learn from others successes and challenges, as well as identify best practices that can be widely imitated.

The DSRIP Transformative piece of the 1115 Medicaid Waiver encourages bringing together the various Performing Providers in the RHP to learn from each other and improve their clinical practices through continuous quality improvement (CQI).

RHP 8’s Learning Collaborative focuses on:

  1. Sharing knowledge and expertise;
  2. Improving patient and care-giver experience; and
  3. Implementing evidence-based guidelines.

RHP 8 Learning Collaborative Events

The Anchor team invites you to visit our new website. We hope you are able to explore the new pages made available that showcase work being done in the RHP 8 Cohort, along with hearing updates and best practices on DSRIP projects in the region that are shared via the monthly conference call.

Regional Work Group

RHP 8 Behavioral Health & Primary Care Cohort

The RHP 8 Behavioral Health & Primary Care Cohort aims to bring together DSRIP Providers and other RHP stakeholders who have similar projects, goals, and interests in identified focus areas.

The objective of the Cohort is to foster collaboration through focused quality improvement activities in an effort to promote efficiencies and improve healthcare delivery and access throughout the region. Learn more about the Cohort.

RHP 8 Lunch & Learn Presentation Series

RHP 8 hosted a learning collaborative presentation on Appreciative Inquiry (AI) in collaboration with RHP 17 on Tuesday, September 23, from noon- 1 p.m. AI is an approach for engaging staff at all levels to produce effective, positive change as a change management and continuous quality improvement (CQI) tool.  Attendees were given information on how they could further explore this quality improvement/change management methodology.
