Learning Collaborative

Collaborative learning occurs when a group of individuals or organizations share information and experiences in an effort to drive change and seek improvement for a common purpose. The RHP 17 Learning Collaborative seeks to achieve regional impact through shared learning, community engagement, and celebration of success. We believe there can be great benefit to the region at large when stakeholders come together in activities designed to foster collaboration and dialogue geared toward learning and continuous quality improvement.

The design of the RHP 17 learning collaborative is a version of the CMS-recommended IHI model, modified to accommodate our smaller, more rural region. The collaborative structure is very similar to that of our sister region, RHP 8 in Round Rock, and our larger regional counterpart in Houston, RHP 3, to allow participation across regions by interested providers.

"Individual" and "Regional" mechanisms for learning and sharing, fed by "Core" components, work in a continuous cycle to promote quality improvement and positive regional impact as illustrated below.

Learning Opportunities

Regional Opportunity: RHP 17 Biannual Learning Collaborative Event

The first biannual learning collaborative event of DY4, sponsored by Faith Mission, will be held at The Cannery Kitchen in Brenham, Texas from 11:30 a.m. to 3:30 p.m. on March 25, 2015. Lunch will be provided by Faith Mission. The event will focus on nutrition in Texas and the role of nutrition in improving health outcomes, with keynote speaker, John Puder, of the Texas Hunger Initiative and presentation spotlights from providers in RHP 17 who are providing nutrition education and services as part of patient care and chronic disease management. Please register by 5 p.m. on Monday, March 16th, by emailing the RHP 17 Anchor Team.

Peer-to-Peer Opportunity: RHP 9 & RHP 10 Collaborative Connections Event

All regional stakeholders and providers are invited to register and attend a joint Learning Collaborative event in Hurst, Texas, hosted by RHPs 9 (Dallas) and 10 (Fort Worth). The event will be held on May 27-28, 2015 at the Hurst Conference Center. This two-day event will highlight a multitude of topics and provide networking with other state providers and community partners. Attendance is free but registration is required. Regional Work Groups

Cohort work groups aim to bring together DSRIP providers and other RHP stakeholders, who have similar projects, goals and interests in identified focus areas. The objective of these groups is to foster collaboration through focused quality improvement activities in an effort to promote efficiencies and improve health care delivery and access throughout the region.

RHP 17 currently has two cohort groups, one of which focuses on behavioral health/primary care and the other focuses on care transitions and navigation.

Initial work group development and organization began in Fall 2014. Volunteer co-facilitators have been leading the group activities with the assistance of the RHP 17 Anchor Team. Both groups are working to establish initial goals and aims.
